REQUIRED DOCUMENTATION:

All previous documentation has been stored or shredded; you will need to provide all documentation for your appointment.

WHAT TO BRING TO YOUR SCHEDULED APPOINTMENT: (These items are necessary to determine your eligibility)

o      Photo ID & Social Security card: for all household members 18 yrs & older. Social security card, Birth certificate or Medical coupon for children under 18.
§    (DOB will need to be provided for all household members.) 

o      Earned Income: for every working household member 18 yrs & older, will need to provide proof of 3 months prior to your appointment, (i.e. Jan. appointments would require Oct. Nov. & Dec) additional Employment Verification Forms may be picked up at OIC Community Services Office. Proof of all check stubs received in the required months. (Pay Dates) (Not pay periods)

o      Any Public Assistance : TANF, SSA, SSI, Pension, V.A, Unemployment, L&I, Child Support or Foster Care that was received in the last 3 months for any and or all family members. (Printouts, Written statements, Copy of checks, Award letters or Bank statements.)

o      Self Employment: Rental Income or Other (Receipts, IRS Filling or Self written statement)

o      Student Loans: Financial Aid Determination

o      Renters: Rental receipt (Made out by Landlord only), Rental Lease Agreement, or OIC’s Housing Status Form, which can be picked up at the OIC Community Services Office.

o      Home Owner/Buyer: Deed, Title, Property tax statement, Purchase contract or Insurance policy.

o      Your most recent Bill statement: for Electric or Gas.

(When scheduling your appointment, please provide the account number & the name of the account holder.)

  An appointment does not guarantee services. Funds & appointments are limited and may run out before your scheduled appointment.
  All required documentation must be brought to your appointment. Failure to bring all documentation may result in delay of services.
   Application process for the Energy Assistance program may take 8-10 weeks to be processed.

DOCUMENTACION NECESARIA 

Toda la documentacion anterior ha sido guardada; Tiene que traer toda su informacion para la cita.

 Lo que debe traer a su cita: (esta informacion es necesaria para determiner si califica)
 

o      ID y tarjeta de Seguro Social: o Acta de Nacimento para cada persona 18 anos y mayor.

Tarjeta de Seguro Social o acta de nacimento o cupon medico para los menor de edad.

o      Ingresos:  para cada persona trabajando en el hogar durante los 3 meses anteriores de su cita. (por ejemplo; Si su cita es en Enero tendria que presentar los ingresos de Oct., Nov., Y Dic.) Talones de cheque o pueden recojer una forma de la oficina del OIC para sus empleadores.

o      Empleo Propio: Ingresos de Renta o Otro: Recibos, delaracion del inquilino o carta escrita)

o      Cual quier clase de Asistencia: Publica, SSA, SSI, Pension, Beneficio Veterano, Desempleo, Compensacion Industrial, Cuidado de Nino o Mantenimiento de Ninos/Esposas que ha recibido el hogar en los 3 meses anteriores: Carta de Beneficios, copia de cheques o carta del banco.

o      Asistencia De Educacion: Carta de la Universidad o recibos de costos escolares.

o      Renta de Casa: Contrato de Renta, ultimo recibo (Del Rentero) o forma de la oficina.

o      Dueno/Comprador: Titulo de Propiedad/casa, Contracto de casa o Comprobante de Impuestos.

o      Bil mas reciente: de la Luz o Gas.

(Favor de tener su numero de cuenta y el nombre a quien esta la cuenta, disponible)

  Una cita no garantiza servicios. Los fondos y citas son limitados y se pueden terminar antes de su cita.
  Debe de traer todos los documentos necesarios a su cita.
  Debe de llegar a tiempo a su cita, si llega tarde puede perder su cita.
  El proceso de su aplicacion para puede durar entre 8-10 semanas.
